TERMS & CONDITIONS
By joining the Deep Dish Junior Beach Volleyball Academy Crystal Palace, you agree to the below:
The person signing this agreement on the behalf of the member who is under 18, has legal responsibility for the child and is entitled to give consent. They also confirm to the best of their knowledge, all information provided on this form is accurate, and will undertake to advise the club of any changes to the information.
All reference to the ‘club’ refers to any facility that Deep Dish operates including its staff, employees, sub contractors, agents and representatives. Facilities refer to the courts and surrounding areas, plus any additional facility within the overall centre of a Deep Dish venue where applicable.
Members of the club are bound, as a condition of membership, to comply with the rules set out below. This is to ensure that the facilities are properly and safely used and that all members have full advantage of them without interfering with the enjoyment of others.
All of the membership rules contained herein apply equally to members, temporary members and guests to our clubs.
I consent to the club photographing or videoing sessions that the child may participate in, to be used for professional, promotional purposes only, only by Deep Dish Beach Volleyball Ltd. If any problems or issues arise, or you do not consent to this, then please contact and we will ensure your needs are met.
All memberships are non-transferrable & non-refundable.
All members must be under 18 years of age to avail of facilities and services provided at the Deep Dish Academy.
Deep Dish Beach Volleyball Academy is a private members club and the management reserve the right to refuse membership to any individual without cause. References may be required on a case-by-case basis and DBS checks will also run on a case-by-case basis. Failure to pass a DBS check may result in membership termination and the refusal of entry again.
Membership may be renewed on payment of the prevailing fee and in accordance with the terms and conditions then in force.
By joining the club, members automatically accept and agree to be bound by these conditions of membership.
The club may withdraw use of all or part of the club for the purpose of undertaking maintenance work or any other work considered necessary.
Entry to the club will only be permitted to those with a valid membership or booking, in date and in credit. If you have an amount due to be paid on your account you will not be able to access the club.
As a member you agree to comply with the rules of the club with regards to use of the facilities, opening hours and your conduct. The club may make reasonable changes to these rules, from time to time, provided the club gives advance notice of these changes.
You may, at your own discretion and expense, obtain personal insurance for loss, injury or damage that you might sustain arising from the use of the club. You participate at your own discretion and accept any injury or illness brought on by exercising or being present at the club is your own responsibility.
Members wishing to report on accidents, incidents, or problems with services at the club should contact a Deep Dish Academy member of staff or email firstname.lastname@example.org in confidence.
In the event of a breach of membership rules and regulations, the club reserves the right of admission and may reserve the right to require any member or guest to leave the premises. Any member found in breach of rules or committing an illegal act, including theft, will be asked to permanently leave the club and be barred from ever entering the club again.
On selecting the course, members will be required to pay a course fee, immediately upon signing up.
Deep Dish reserves the right to cancel membership if payment is not received. Access to the club will be denied until said payment is received and if payment is not received.
Memberships are non-transferable and non-refundable.
Hours Of Operation
The club reserves the right to vary the opening hours of the facility and club sessions considered necessary for the proper operation of the club.
Safety & Hygiene
Throughout the course of your membership with the club, regular health and medical screening should be sought from your general practitioner.
Before using the club, you should have permission by your doctor to participate in high intensity exercise. Should you have any questions about the equipment at the club, please ask the closest member of staff to show you how to safely use. This is your own responsibility.
It is necessary that all members and guests of the club wear appropriate, clean attire when using the club.
If you feel dizzy, faint, unwell, or feel any unusual pain then you must stop exercising and inform a member of staff immediately.
Members must make the club aware of any previous medical issues on their registration form. They must also make coaches aware of any injuries, pains or concerns prior to the session taking place. It is however the member’s choice to participate and no liability will be placed on Deep Dish or its staff for injury, illness or death.
As a parent or guardian of the child, I give my consent that in an emergency situation, the club may act in my place, if the need arises for the administration of emergency first aid and/or other medical treatment which, in the opinion of a qualified medical practitioner, may be necessary. I also understand that in such an occurrence all reasonable steps will be taken to contact me or the alternative adult which I have named in the form. I confirm to the best of my knowledge, my child/the child in my care does not suffer from any medical condition other than those detailed by me in this form.
